This form needs to be printed and filled out. We ask that you start by taking the full version of the APEST test which can be found by going to https://fivefoldministry.com/. When you've completed the APEST test, you should record your results on the membership form. After you've completed the membership form, please schedule your meeting with the Elders. Be sure to bring your completed form with you for your interview with the Elders. The Elders will approve those qualified for membership.
